1. The OH! Card program is a multi-benefits health loyalty card program (“Program”) governed by the terms and conditions issued by the program owner and manager, Tigas Pharma (M) Sdn. Bhd. (497299-V) (“Tigas”) located at the full address below.
2. All applicants for the OH! Card must be at least 18 years of age and have a valid Malaysia residential address. The OH! Card is valid for 12 calendar months of issue, unless as stipulated on the reverse side of the OH! Card under the column “Expires”. OHpoints have a 36-month validity from date of issue for redemption.
3. The OH! Cards and OHpoints are issued by and will remain the sole property of Tigas. By utilizing the OH! Card and accepting the issuance of OHpoints, the OH! Cardholder implicitly accepts and agrees to use the OH! Card in accordance to the terms and conditions contained herein.
4. The OH! Card has to be activated in order for the OH! Cardholder to accumulate OHpoints or enjoy merchant offers. Web portal activation applies to Tigas pharmacy store-based OH! Card issuance only.
5. OHpoints may only be earned for purchases of goods and services at all Tigas pharmacy stores. Visit www.tigas.biz for complete pharmacy store locations and opening times. “Tigas” signboards and decals at pharmacy shop fronts are used as identification points of acceptance of the Program.
6. OH! Card may be also be used at nominated third party partners and merchants for flashcard discounts in accordance to Tigas’ promotional tie-ups. List of merchants and current offers are published on www.onlyhealth.com.my, the OH! Privileges mailer (OH! Yes Newsletter) and OH! Health magazine. Tigas will not be liable for any inconsistency of the details (by the selected third party partner merchants) of the promotions (discounts or privileges) as advertised in any type of media.
7. The OH! Card must be presented at the Tigas pharmacy point of purchase in order to enjoy the offers and/or accumulate OHpoints. A fee of RM10 will be charged by Tigas to replace lost or damaged cards.
8. The OH! Card is not a credit card and no credit facilities will be offered to the OH! Cardholder. The OHpoints accumulated at all Tigas pharmacy stores do not have any cash value and cannot be redeemed or exchanged for cash.
9. The OH! Cardholder shall enjoy 100 welcome OHpoints and 100 bonus OHpoints upon successful activation of card via web portal (for Tigas store issued cards only). OHpoints may also be recorded in the OH! Cardholder account in relation to promotion and incentive programs.
10. The OH! Cardholder will earn 1 OHpoint for every RM1 spent. The OHpoints shall be calculated on the NETT value of the transaction and instead of up to the nearest dollar. The OHpoints accumulated in the OH! Cardholder's account is not transferable.
11. Selected redemptions will be made available with OHpoints funding of 1.5% on OHpoints i.e 1,000 OHpoints conversion to RM15 worth of products, and/or Tigas Voucher, upon the individual confirmation with individual Tigas stores for sufficiency of points and funding amount. Redemptions may be made using the various redemption methods implemented by Tigas.
12. Product(s) once redeemed, is/are not exchangeable or refundable. Product(s) must be redeemed within 36 months from date of issuance of such OHpoints or any other extended period as stated in the OH! Privileges mailer (OH! Yes Newsletter), other promotional literature and/or on-line via www.onlyhealth.com.my. Redemptions will be forfeited should the OH! Cardholder fails to redeem the products within the stated period.
13. No reservations on product(s) is/are available for redemptions. Product(s) is/are available on a first come first serve basis, whilst stocks last. Tigas does not warrant the efficacy or quality of products that are redeemed in Tigas pharmacy stores.
14. OH! Cardholder will be invited to events, roadshows and health forums conducted by Tigas as stipulated under the Program.
15. Tigas reserves the right to decline application, re-issue or withdraw the OH! Card without providing reasons at any time. Tigas is entitled, for any reason, and at any time without liability or notice, to suspend the calculation and accrual of OHpoints, to rectify any errors in the calculation or otherwise adjust such calculation.
16. Tigas is also entitled, for any reason, and at any time without liability or notice, vary modify for amend any of the terms and conditions or terminate the Program, as it may, in its absolute discretion think fit and the OH! Cardholder shall be bound by such variations, amendments and/or termination.
17. Tigas respects the privacy of the OH! Cardholder. The OH! Cardholder agrees to disclose his/her personal data (as defined in the Personal Data Protection Act 2010) to Tigas (and its nominated contractors and business partners) solely for the purposes of data processing and where necessary for the delivery of such prescribed benefits under the Program. The OH! Cardholder may elect not to disclose his/her personal data at any time to Tigas, whereupon Tigas will be unable to fulfill the elements of the Program, including the awarding and redemption of the OHpoints.
18. Tigas may, from time to time, send out notifications on the Program benefits vide various modes of communication channels as consented hereto by the OH! Cardholder. The OH! Cardholder is entitled, at all times by written notice to Tigas, nominate not to receive such promotional materials and opt out of any and/or all forms of communication. This opt-out option shall be available on all digital communication modes deployed by Tigas.
19. The OH! Cardholder is entitled to update his/her personal data which is kept and maintained by Tigas via access to the web portal. Access to the web portal will be subject to password and member log-in authentication. Tigas will take all practicable steps to ensure security of the data from unauthorized access.
20. Upon expiry of the OH! Card, and/or OHpoints, the OH! Cardholder may, by his/her individual election nominate to continue his/her membership by subscribing the renewed terms of the Program.
